Request Form

Instructions:

1. User must be using Adobe Reader 9.0 and later.

2. You can download Adobe Reader here.  Follow Adobe's website instructions for installing Adobe Reader 9.0.  (You might want to remove previous versions of Adobe Reader before installing Reader 9.0)

3.  Next you will need to download the Custom Print Request Form

4.  You will want to download this and save it to your computer.

5.  If it tries to open the PDF in a web browser, go to file and click Save As:  Save it to your desktop for easy access.

6.  Once it is saved, open the file in Adobe Reader 9.0, fill out Custom Request form and hit the submit button. After hitting submit you should see a message asking for your Email Address and Name.  You can use any email address you want and please use your Full Name as this will help us in tracking who the responses come from.

7.  After hitting send, you will get a "Your Response has been queued for submission"

8.  Shortly after you will get another message stating: "Your response was successfully sent"

9.  At this time you can submit another Custom Print Request.

10.  After exiting the Form, click No so that changes to the form are not saved otherwise the changes you made will be there when you open the form again.
